29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: a) On or about July 31, 2019, at the east and west side of bld. 4413, the three level scaffold used by employees to do stucco work was missing base plates and mudsills at different vertical components, exposing employees to fall and struck-by hazards.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) On or about July 31, 2019 and at times thereafter, at the east and west side of building 4413, the employer did not ensure that employees erecting and dismantling a three level scaffold were directed by a competent person in scaffolding. The three level scaffold was missing all mudsills and baseplates, exposing employees to fall and struck-by hazards.
