29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about 8-6-2019, in the RXO lab and Swarf production area at 101 Greenwood Industrial Blvd, McDonough, GA: Employees performing servicing and maintenance activities on the equipment and machines, were exposed to amputation and caught-in hazards, in that; no procedures were developed or utilized to control the electrical, heat, and gravitational energy sources such as but not limited to the following equipment.  a. Employer did not developed procedure - OPTO Tech Taper OTT 80 CAN-A Schneider - Electrical and heat energy sources b. Employer did not developed procedure- Swarf equipment - Electrical, water, and high pressure energy sources c. Employer did not developed procedure- Transfer Pumps and Filter system HSC Lines - Electrical, heat, water pressure energy sources d. Employer did not enforce utilization of procedure -  CCU Modulo Schneider Electrical, pneumatic, water, and heat sources e. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 8-6-2019, in the RXO lab and Swarf production area at 101 Greenwood Industrial Blvd, McDonough, GA:  Authorized and affected employees performing servicing and maintenance on the equipment and machines not limited to the ones listed below, were exposed to amputation and caught-in hazards.  The last documented periodic inspections modification to meet plant specifics was 4/17/2018.  a. OPTO Tech Taper OTT 80 CAN-A Schneider - Electrical and heat energy sources b. On or about 8-6-2019, in the RXO lab and Swarf production area at 101 Greenwood Industrial Blvd, McDonough, GA:  Employees performing servicing and maintenance activities on the equipment and machines, were exposed to amputation and caught-in hazards, in that in that they were not aware of the electrical, heat, mechanical, and gravitational energy sources or how to isolate the sources in equipment such as but not limited to equipment listed below.  a.
