29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition:  a) Luxottica Group, McDonough, GA - In the laboratory area (Modulo), employees were exposed to an ingestion hazard where personal protective equipment such as, but not limited to face shields, aprons, gloves, etc. were stored on top of lead/cadmium alloy pots, deblockers, and sponges (removing slag). Wipe samples of an employees' face shield stored on this cart contained 159.4 micrograms and 82 micrograms (ug) of lead.  These face shields also contained 107.3 micrograms and 26.2 micrograms of cadmium, on or about August 6, 2019.    These face shields were also shared between employees without an adequate cleaning schedule.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1025(g)(1): Appropriate protective work clothing and equipment was not provided when employee(s) were exposed to lead above the permissible exposure limit (PEL), without regard to the use of respirators, or where the possibility of skin or eye irritation exists:  a) Luxottica Group, McDonough, GA - In the laboratory area, employees were exposed to skin irritation hazards in that they are required to reclaim and/or add liquid alloy containing 22.6% lead and 5.3% cadmium.  Employees were observed using latex gloves that that were not long enough to prevent splashing of the forearms when reclaiming alloy and other jobs, on or about August 6, 2019.    b) Luxottica Group, McDonough, GA - In the laboratory area, DBA Deblocking employees were exposed to skin irritation/lead splash hazards in that several employees' latex gloves were observed torn while cleaning and emptying the deblocker with lead/cadmium alloy creating a skin irritation hazard.  The employer did not adequately access the PPE for alloy handling, on or about August 6, 2019.  c)Luxottica Group, McDonough, GA - In the laboratory area, Hot Flash employees were exposed to skin burn hazards in that the latex gloves used to empty lead alloy was not adequate to protect against burning their skin. The employer did not adequately access the PPE for alloy handling, on or about August 6, 2019.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Luxottica Group, McDonough, GA - Maintenance employees are required to repair equipment that contains hazardous chemicals such as, but not limited to lead/cadmium alloy holding the heated product. Training, to include PPE and hazards as required by the standard was not conducted, on or about August 6, 2019.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
