Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible wood dust deflagration propagation hazards while working at indoor workstations upstream of an outdoor dust collector: Production Area: On or about August 8, 2019, employees were exposed to combustible wood dust deflagration propagation hazards, including exposure to a propagating flame front, while working at woodworking equipment located upstream of an outdoor AAF Amer Pulse (Model 10-144) dust collector. The dust collection system lacked a means of deflagration propagation protection to isolate the dust collector from the upstream equipment/process.
