29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  a) Construction site, The Promenade at Upper Dublin, Colum line N to R and 1 to 3  - On or about August 8, 2019, employees rigged a horizontal lifeline using a retractable lanyard that looped around a beam and was hooked back onto the cable. The employee then used a second retractable to attach to the first. A qualified person had not determined the potential forces or free fall distance to maintain the required safety factor.
