29 CFR 1926.152(e)(4): Units dispensing flammable or combustible liquids were not protected against collision damage: a) Jobsite, 3906 Logans Ferry Road: On or about August 8, 2019, a 2,000 pound diesel fuel tank was located by the entrance to the jobsite, within 3-4 feet of the job trailer and was not protected against damage cause by the approximately nine pieces of excavation equipment onsite and in the vicinity of the tank.

29 CFR 1926.652(c)(3)(ii): The tabulated data was not in written form and include all of the following: a) Jobsite, 3906 Logans Ferry Road, Monroeville, PA: On or about August 8, 2019, the employer provided one document for the company owned 20' x 8' trench box, serial number 00101 that was onsite that showed the dimensions and measurements and the engineer stamp, but did not include the written identification of the parameters that affect the selection of a protective system drawn from such data, identification of the limits of use of the data and explanatory information as may be necessary to aid the user in making a correct selection of a protective system from the data.
