29 CFR 1910.30(a)(3)(iv): (iv) The employer must ensure the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of the training section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer:    a)  Appliance Factory Mattress Kingdom, 321 W 84th Ave, Thornton, CO  80260:  On and before August 12, 2019, the employer did not ensure that employees were trained on the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of the training section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  Employees using cherry pickers were not trained on the proper equipment to use to prevent contact with a lower level.  This condition exposed employees to fall hazards.

29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee cannot free fall more than 6 feet nor contact a lower level:    a)  Appliance Factory Mattress Kingdom, 321 W 84th Ave, Thornton, CO  80260:  On and before August 12, 2019, the employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee cannot free fall more than 6 feet nor contact a lower level.  Employees used shock absorbing lanyards when using the cherry pickers, and this equipment did not protect employees from striking the lower level.  This condition exposed employees to fall hazards.

29 CFR 1910.176(b): Storage of material was not secure, creating a hazard:    a)  Appliance Factory Mattress Kingdom, 321 W 84th Ave, Thornton, CO  80260:  On and before August 12, 2019, the employer did not ensure that storage of product on shelves was secure.  A forklift had damaged the leg frame of a storage rack, creating an unsecure base where stored product, including appliances, could slide and collapse.  This condition exposed employees to crush-by hazards.

Forklift operators were observed exceeding the posted forklift speed limit of 4 miles per hour.  This condition exposed employees to struck-by hazards.
