29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  (a) A and A Quality Appliance, Inc. at 321 W 84th Ave., Thornton, CO 80260: On and around February 22, 2021, an electrical outlet located on a wall in the Showroom was missing an outlet plate cover. The employer did not ensure that live parts operating at 50 volts or more were guarded against accidental contact by utilizing an outlet plate cover. This condition exposed employees to hazards associated with shock and fire.

29 CFR  1926.405(j)(1)(i):Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact:  (a) A and A Quality Appliance, Inc. at 321 W 84th Ave., Thornton, CO 80260: On and around February 22, 2021, the controlling, creating, correcting, and exposing employer did not ensure that an electrical outlet located on a wall in the entrance to the Showroom had an outlet plate cover. The outlet plate cover had been removed  to paint the wall. Equipment that was being used for the paining work was plugged into the energized, uncovered electrical outlet. This condition exposed employees to hazards associated with shock and fire.

Employers shall provide employees with effective information and training on any operations in their work area where hazardous chemicals are present; and the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets required by this section.  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) A and A Quality Appliance, Inc. at 321 W 84th Ave., Thornton, CO 80260: On and around February 22, 2021, the employer did not ensure that employees were provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment. Employees did not know the hazards of the chemicals in their work areas or the details of the program, including labelling and safety data sheet requirements. This condition exposed employees to chemical hazards, including but not limited to Goo Gone and vehicle exhaust containing carbon monoxide.   A and A Quality Appliance, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1910.1200(h)(1) which was contained in OSHA Inspection Number 1442257, Citation 1 Item 1 Group Letter b issued on November 21, 2019 and became Final Order on December 17, 2019 with respect to a workplace located at 321 W 84th Ave., Thornton, CO 80260.

This condition exposed employees to electrical hazards while connecting and disconnecting appliances at the outlet boxes.  (b) A and A Quality Appliance, Inc. at 321 W 84th Ave., Thornton, CO 80260: On and around February 22, the employer did not ensure that a metal electrical outlet box was used in accordance with listed instructions, in that the outlet box that was fastened to a wall included pre-punched knockouts that could be dislodged by frequent handling or being struck-by objects in the Showroom. This condition exposed employees to electrical hazards while connecting and disconnecting electric-powered equipment at the outlet box.
