OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation injuries while operating a compound miter saw that was set-up on an unstable surface. An employee operated a 10 inch Metabo HPT Compound Miter Saw (SN CN8 11031) to cut one foot sections from a 16 foot long, two by four board while the saw was sitting on a pallet of stacked rolls of plastic. The employee was attempting to hold the long end of the board with his left hand while operating the saw with his right hand. There was no method used to secure the long end of the board while the cuts were being made. The saw was not being operated on a stable working surface and the board jammed in the rotating blade pulling the employee's left hand toward the blade which resulted in an amputation injury to the employee's thumb. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records
