29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  PRODUCTION AREA, EXTRUDER LINE 2 - Employees operating extruder line two were required to clear clogs in the starve feeder, and the employees were not required to perform lock out tag out to shut off the rotating auger that feeds the material into the extruder prior to clearing the clog, on or about January 10, 2020.    (b)  PRODUCTION AREA - Employees serviced and repaired plant equipment and machinery, and the employer had not developed written lock out tag out procedures for the plant machinery that had more than one energy source, on or about January 10, 2020.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  (a)  FACILITY WIDE - An employee sustained a severe finger laceration injury that resulted in an in-patient hospitalization on January 10, 2020, and the employer did not report the event to OSHA until January 12, 2020.  No abatement certification or documentation required.
