29 CFR 1910.141(c)(1)(iii): The sewage disposal method endangered the health of the employee(s) and the washing facilities were not maintained in a sanitary condition:    Taylor Pallets & Recycling, Inc., Winder, Ga - On or about May 30, 2019, employees were exposed to unsanitary conditions  in the men and women bathrooms used by some employees:     a)toilets in the men/women shop area were not flushing and congested with feces,     b)there was no water in one hand washing facility and only cold water in the other hand washing facility.  The washing facilities were not maintained in a sanitary condition.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The employer failed to maintain and test the emergency lighting fixtures on a periodic basis to ensure the lights would function properly during a facility power failure, on or about May 30, 2019. Additionally, the line-of-sight to an exit sign was not clearly visible at all times:    Taylor Pallets & Recycling, Inc., Winder, GA- In the warehouse area, employees were exposed to a fire hazard when the employer did not have signs or other marking that provided clear directions to a safe evacuation route for exiting the warehouse area.  The line of sight to an exit was not clearly visible due to stacked boxes and material, on or about May 30, 2019. The employer did not ensure that daily inspections were performed on the forklifts before placing it in service to detect any deficiencies/defects, exposing employees to struck by hazards in the following:    a)Toyota sit down forklift #1 - The back-up warning alarm system and lights were inoperable and/or missing and the seat belt was inoperable/broken.  b)Toyota sit down forklift #3 - The back-up warning alarm and lights were inoperable and/or missing and the horn did not work.   c)Toyota sit down forklift #6 - The back-up warning alarm was inoperable and/or missing and the horn did not work.  d)Toyota sit down forklift #8 - The back-up warning alarm and lights were inoperable and/or missing, the horn was inoperable, and the seat belt was inoperable/broken.    The employer did not remove defective forklifts from service.    The Taylor Pallets & Recycling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 1314993, citation number 1, item number 2 and was affirmed as a final order on 07/02/2018, with respect to a workplace located at 1030 Bankhead Hwy, Winder, Ga.
