29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Bush Hog Facility: On August 15, 2019 the employer did not report an in-patient hospitalization of an employee as a result of a work-related incident within twenty-four (24) hours after the employee was hospitalized. An employee was taken to hospital from the work place on August 15, 2019 at about 6PM for work related incident. The employee called a Supervisor on August 15, 2019 at about 10PM to notify him that we was being hospitalized.
