29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about 8/28/2019, at jobsite located at 309 SE 5th St, Dania Beach, FL 33004, the employer did not ensure that safety operating procedures were followed for clearing cut-grass from discharge chute of a riding lawnmower (i.e. eXmark Lazer Series S-type, Model LZS749EKC60400, Serial #315658119), exposing an employee to an amputation hazard.       In the Alternative:    OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to an amputation hazard:    On or about 8/26/2019, at jobsite located at 309 SE 5th St, Dania Beach, FL 33004, the employer did not ensure that safety operating procedures were followed for clearing cut-grass from discharge chute of a riding lawnmower (i.e. eXmark Lazer Series S-type, Model LZS749EKC60400, Serial #315658119), exposing an employee to an amputation hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about 8/28/2019, at the mechanic shop of Dynaserv Florida LLC, 990 S Flamingo Rd, Davie, FL 33304, employees were exposed to an electrical hazard when connected a fan (i.e. Model No. SFD1-500, Serial No. 1002770501) to an extension cord that lacked strain relief at the female end attachment plug.
