This was most recently documented on or about August 28, 2019 at 14700 Bailie Street, Waverly, NE 68462. Extension cords utilized by employees had sections of insulation that were damaged/worn through exposing the conductors within.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used:  The employer is failing to ensure that all employees are not exposed to electrical shock hazards while utilizing extension cords attached to power tools. This was most recently documented on or about August 28, 2019 at 14700 Bailie Street, Waverly, NE 68462. Extension cords utilized by employees had sections of insulation that were damaged/worn through exposing the conductors within.

This was most recently documented on or about August 28, 2019 at 14700 Bailie Street, Waverly, NE 68462 when employees were observed installing sheeting on a residential structure while working at greater than 6 feet above the ground without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer is failing to train all employees potentially exposed to fall hazards to enable each employee to recognize the hazards associated with the type of work being conducted. This was documented on or about August 28, 2019 at 14700 Bailie Street, Waverly, NE 68462.  This should include training on mitigating those hazards through an acceptable method of fall protection. Employees were observed performing framing activities having not received fall protection training.
