29 CFR 1910.25(b)(5)(i): When a door or gate opens directly on a stairway, a platform was not provided and/or the platform did not provide a usable depth greater or equal to 20 inches:    (a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056:  On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located at the southern stairway leading to the second floor of the west wing of Building 590.  When the door was opened, the platform usable depth was approximately 11 inches.    (b) Hill Air Force Base at 7290 Weiner Street Building 383: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located at the southeast entrance of the building measured a platform depth of approximately 18 inches.     (c) Hill Air Force Base at 7290 Weiner Street Building 383: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located at the southwest entrance of the building measured a platform depth of approximately 15.5 inches.     (d) Hill Air Force Base at 54 Dogwood Avenue Building 1255: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located on the exterior south side of the building had a usable platform depth of 5.5 inches.    (e) Hill Air Force Base at 6046 Cedar Lane Building 1276: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located on the northwest side entrance had a usable platform depth of 17 inches.    (f) Hill Air Force Base at 6046 Cedar Lane Building 1276: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located on the southeast side entrance had a usable platform depth of 0 inches.    (g) Hill Air Force Base at 6046 Cedar Lane Building 1276: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located on the south side entrance had a usable platform depth of 16 inches.    (h) Hill Air Force Base at 6016 Cedar Lane Building 1279: On or before August 29, 2019, the employer did not ensure that when a door opens directly on a stairway, the usable depth was greater than or equal to 20 inches. The doorway located on the north side of the building had a usable platform depth of 15.5 inches.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2)    (a) Hill Air Force Base at Building 238: On or before August 29, 2019, the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway located within the building interior bay at door 1EC on the north east corner of the building facing South Gate Avenue consisted of at least 3 treads and 4 risers and did not have a stair rail system on the open side of the stairway. This exposed employees using the stairway to fall hazards.     (b) Hill Air Force Base at 6062 Fir Avenue Building 1234: On or before August 29, 2019, the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway located on the exterior south west side of the building consisted of at least 3 treads and 4 risers and did not have a stair rail system on the open side of the stairway. This exposed employees using the stairway to fall hazards.

29 CFR 1910.23(d)(13)(i): The employer did not ensure that fixed ladders that do not have cages or wells have a clear width of at least 15 inches on each side of the ladder centerline to the nearest permanent object:  a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that the fixed ladder located on the second floor hallway of the west wing maintained a clear width of 15 inches from the centerline of the ladder rungs to the nearest permanent object. A celling tile had been removed so that employees could gain access to equipment located above the ceiling. The width of the ceiling tile was measured to be 23 inches, and had a clear width of approximately 11.5 inches from the centerline of the ladder rungs, thus not providing 15 inches of clear space from the centerline of the ladder rungs. This exposed employees to fall hazards when climbing the fixed ladder.

29 CFR 1910.28(b)(11)(i): Each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet or more above a lower level was not protected by a guardrail or stair rail system:  a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that the stair rail system, located on the upper flight leading to the second floor interior of the west building wing, had a midrail installed. The lack of a midrail in the stair rail system exposed employees to fall hazards of greater than 10 feet.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that all exit discharge signs were visually apparent in Building 590. The exit sign located on the north side of the second floor of building 590 was installed parallel to the walkway of the hallway which makes it difficult to visually see the sign when standing in the middle of the hallway. This exposes employees to hazards due to lack of signage in a fire exit pathway.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that all fire extinguishers were mounted on the second floor of Building 590. Two fire extinguishers were placed on the ground of in the hallway of the second floor of Building 590 in case they would need to be used for any reason while the fire suppression system was being repaired. This condition exposed employees to fire hazards due to fire extinguishers not being mounted, located, and identified.

Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that electrical panels were fully protected from damage. An electrical panel located on the second floor in the southern office area had been damaged from the room door swinging and repeatedly hitting the panel. Engineering controls have not been implemented to prevent damage from occurring to the electrical panel.

29 CFR 1910.304(b)(2)(iv)(B): Ground-fault circuit-interrupter protected receptacles were not be provided where replacements are made at receptacle outlets that are required to be so protected elsewhere in this subpart:  a) Hill Air Force Base Lamn Ln. Building 590 at Hill Air Force Base, UT 84056: On or before August 29, 2019, the employer did not ensure that non grounding type receptacles were replaced with ground fault circuit interrupter protected receptacles in the southern office area of the west building wing. Two non grounding type receptacles located within six feet of a sink were not GFCI protected. This exposes employees working in the area to electrical hazards.
