29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.    (a) On or about September 3, 2019, in the machine finishing area located at 4021 South Kinnickinnic Ave, St. Francis, Wisconsin, 53235, where employees used polishing machines to spot finish manufactured product, surfaces were covered with an excessive layer of metal dust.  These surfaces included the walls, pipe and exhaust duct runs, polishing wheels hanging on racks on the walls, and on the polishing equipment.  This condition potentially exposed employees to a fire hazard.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  On or about September 3, 2019, the employer did not develop and establish a written hazardous energy control program (Lockout/Tagout) which exposed employees to caught-in and crushing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  On or about September 3, 2019, the employer did not ensure that rotating parts on, but not limited to the Hammond Machinery Builders and D Gardner Machine Co. metal polishing machines were properly guarded. This exposed employees to in-going nip points, rotating shafts and amputation injury.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) On or about September 3, 2019, the employer did not develop and implement a hazard communication program, that would describe or include at least the following:     1) Requirements for labeling of containers of hazardous chemicals;    2) Safety Data Sheet (SDS) availability;    3) Training of employees;    4) A complete list of hazardous chemicals known to be in the workplace;    5) Methods to inform employees of the hazards of non-routine tasks; and    6) Methods to inform other employer(s) of safety data sheet availability; the labeling system; and any precautionary measures to protect employees.    Workers were exposed to hazardous chemicals and combustible dust - fire, deflagration and explosion hazards.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.    (a) On or about September 3, 2019, employees were not trained on the combustible nature of the dust produced from polishing operations, including ability to catch fire, deflagrate or explode.
