29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Masin Roofing Inc., on or about September 5, 2019 at the address of 715 Monroe Street, Toledo Ohio: The employer did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial type structure. Employees installing new sheeting and roofing material from atop the structure were exposed to falls of approximately 33 feet to the ground below. Masin Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(11), which was contained in OSHA inspection number 1239735, citation number 1, item number 1 and was affirmed as a final order on August 01, 2017, with respect to a workplace located at 4068 E. Harbor Rd. Port Clinton, Ohio, 43428.
