29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):     a) Shop Area: An employee, Welder , was exposed to continuous noise at 84.4% of the permissible exposure level of 90 dBA or an eight-hour time-weighted average of 88.7 dBA. This exposure was observed over a 458 minute sampling period on September 10, 2019. The employee was not trained on the effects of noise or the purpose of the audiometric testing, on or about September 10, 2019.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replace:  a) Welding Area: Employees MIG welding used grounding cables that were damaged and not replaced, on or about September 10, 2019.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) Shop Area: Employees wore elastomeric half face respirators on a voluntary basis while welding and the employer did establish a written program including medical evaluations and storage requirements, on or about September 6, 2019.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
