29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge. (a) Betsy Ross Barge - Manitowoc 4100W (serial 41854) - On or about September 11th, 2019 and at times prior thereto, employees were exposed to a fall of 3' and 6' while transiting the two (2) wooden stairways to access the Manitowoc 4100 crane, neither of which had adequate handrails.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. (a) Work Barge "Betsy Ross" - On or about September 9, 2019 and at times prior thereto, an employee was exposed to a fall of approximately 10' while working in an aerial work platform (JLG model 660 SJ serial 856108) without adequate fall protection training.
