The grinder did not have a guard; on or about 09/12/2019.   Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.243(c)(5)(i): All wheels were not closely inspected and sounded by the user to ensure they have not been damaged in transit, storage, or otherwise immediately prior to mounting. The spindle speed of the machine was not checked before mounting of the wheel to ensure that it did not exceed the maximum operating speed marked on the wheel:  a) Worksite, 40 Ranick Rd, Hauppauge, New York - An employee was authorized to operate a Master Mechanic 4 1/2in Angle Grinder Model No. 211886 with a spindle speed of 12,000 RPM. Mounted on the spindle was a 7in Harbor Freight Tools Warrior Cut of Wheel with a max operating speed of 8,500 RPM. The spindle speed was not checked prior to mounting the Cut-Off Wheel and was not inspected and sounded prior to use; on or about 9/12/2019.  Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
