29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure: a) Worksite, 65 Commerce Dr., Hauppauge, New York: On or about November 05, 2020, an employee unjamming a ROAN American Liberty Series Side Weld Bag Making Machine was not trained on the purpose and use of the energy control procedures. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19. Poly Craft Industries Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(i)(B, which was contained in OSHA inspection number 1368403, citation number 1, item number 3 and was affirmed as a final order on 04/06/2017 , with respect to a workplace located at 40 Ranick Road, Hauppauge, New York.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Worksite, 65 Commerce Dr., Hauppauge, New York: On or about November 05, 2021, affected employees were unjamming a ROAN American Liberty Series Side Weld Bag Making Machine and a lockout or tagout devices were not affixed to each energy isolating device by an authorized employee. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
