29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   A)     Greif, Inc., located at 4031 Spartan Drive, Oregon, OH 43616: On or about September 17, 2019, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from the hazards such as, but not limited to, nip points between the rotating roller and stationary table of the conveyor adjacent to the Line 3 Spiral Tech Saw. Employees were exposed to hazards associated with unguarded nip points.

29 CFR 1910.212(a)(2): Machine guard(s) on machines were so installed that they offered accident hazards in themselves:   A)     Greif, Inc., located at 4031 Spartan Drive, Oregon, OH 43616: On or about August 19, 2019, the employer did not ensure that the guard, located on the Spack System Cutter (Model: Spackliner 25-400; Serial # 1155), was maintained in a manner not to pose a hazard in itself. The employer did not repair the damaged guard of the Spack System Cutter. Employees operating the Spack System Cutter in this condition were exposed to laceration hazards.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   A)     Greif, Inc., located at 4031 Spartan Drive, Oregon, OH 43616: On or about August 19, 2019, the employer did not report a personal injury incident when one employee was admitted to and treated at the hospital due to a severe laceration of the right arm. The employer or employer representative did not contact the OSHA Toledo Area Office or the 24-hour OSHA number to notify the agency of a personal injury incident, resulting in the employees hospitalization and treatment, within 24 hours of the occurrence. The incident happened on August 19, 2019, at approximately 13:30 hours, and the injured employee was admitted to and treated at the hospital the same day.
