29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On October 8, 2019 at the blaster, one or more methods of machine guarding was not provided to protect employees from moving parts such as the mechanical bar used to move the beams horizontally and vertically on the steel beam table.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. (a) On October 8, 2019 at the blaster, the employees were exposed to ingoing nip points, in that the sprocket wheels and chains were not enclosed for the rollers on the conveyor which were powered by the blaster BCP Products Wheelabrator Model DEA01990 SN 2727 and less than 7 feet above the floor.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On September 16, 2019, the employer had not reported within 24 hours the amputation of a finger, as a result of a work-related accident to OSHA. The accident occurred on September 12, 2019, and it was reported to OSHA on September 16, 2019.
