29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  JOB SITE: 5319 S. Bellerieve Lane, Imperial, MO 63052, on or about 10/1/2019, workers were observed on to the roof, operating pneumatic nail guns, actively installing new roof material, without any form of eye or face protection.  The workers were exposed to struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE: 5319 Bellerieve Lane, Imperial, MO 63052, on or about 10/1/2019, workers were observed on top of a 6:12 pitch roof, actively installing new roof material, approximately 10 feet, 11 inches eave to ground level, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.
