29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 08/29/19 at 465 Winn Way, Decatur, GA - Employees were exposed to a fall hazard while replacing shingles without the use of personal fall protection at heights greater than 15 feet.     No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 08/29/19 at 465 Winn Way, Decatur, GA - Employees were exposed to a fall hazard while replacing shingles without the use of personal fall protection at heights greater than 15 feet. The employer failed to develop and implement a fall protection training program to recognize and minimize fall hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 08/29/19 at 465 Winn Way, Decatur, GA - The employer failed to ensure that a ladder used by employees to repair a roof did not extend at least 3 feet above the roof surface and/or was not secured at the top as required while at heights of approximately 15 feet.   No abatement certification or documentation is required for this item.
