The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Alejandro Perez Rodriguez located at 1710 Wilshire Drive, Upper Arlington, OH 43220: On and preceding October 3, 2019, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were installing shingles with pneumatic nail guns on a roof that varied from 10-19 feet. The employees were not utilizing proper eye, head or fall protection which exposed them to falls up to 19 feet and struck by hazards to the head and eyes.

Roofers were installing roofing shingles utilizing a pneumatic nail gun set at 100 psi and were not utilizing eye protection which exposed them to struck-by hazards to the eyes.
