29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. Located at 3386 Creekside Trail Drive in Canal Winchester, Ohio: On or about December 22, 2021, the employer did not maintain an effective safety and health program to ensure hazards were identified and corrected onsite while employees performed roofing work.  Alejandro Perez Rodriguez was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1538667, citation number 2, item number 1 and was affirmed as a final order on August 24, 2021, with respect to a workplace located at 112 Claytonbend Drive, Pataskala, OH 43062.  Alejandro Perez was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1485797, citation number 1, item number 1a and was affirmed as a final order on October 16, 2020, with respect to a workplace located at 3380 Noreen Drive, Columbus, OH 43221. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at 3386 Creekside Trail Drive in Canal Winchester, Ohio: On or about December 22, 2021, employees were performing roofing work from a two-story house without fall protection, thereby exposing employees to fall hazards from approximately twenty feet.  Alejandro Perez Rodriguez was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1538667, citation number 2, item number 3 and was affirmed as a final order on August 24, 2021, with respect to a workplace located at 112 Claytonbend Drive, Pataskala, OH 43062. Located at 3386 Creekside Trail Drive in Canal Winchester, Ohio: On or about December 22, 2021, employees were operating pneumatic nailers to fasten shingles to the roof without eye protection, thereby exposing employees to a struck-by hazard.
