On or about June 25, 2021, located at 112 Claytonbend Drive, Pataskala, OH 43062, the employees used an unsecured ladder placed on the ridge of a lower section of gable roof to access the upper section of gable roof, thereby exposing employees to a fall hazard of greater than 10 ft.

On or about June 25, 2021, located at 112 Claytonbend Drive, Pataskala, OH 43062, the employer did not maintain a safety and health program to ensure detection and correction of hazards during roofing activities.  The Alejandro Perez was previously cited for a violation of this occupational safety and health standard 1926.20(b)(1), which was contained in OSHA inspection number 1485794, citation number 1, item number 1a and was affirmed as a final order on October 16, 2020, with respect to a workplace located at 3380 Noreen Drive, Columbus, OH 43221.  The Alejandro Perez Rodriguez was previously cited for a violation of this occupational safety and health standard 1926.20(b)(1), which was contained in OSHA inspection number 1435234, citation number 1, item number 1 and was affirmed as a final order on November 13, 2019, with respect to a workplace located at 1710 Wilshire Drive, Upper Arlington, OH 43220.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about June 25, 2021, located at 112 Claytonbend, Pataskala, OH 43062, the employer did not ensure that employees involved in roofing activities on a two-story, single family home were protected against fall hazards. Three employees were installing roofing felt paper on the roof without fall protection, thereby exposing them to fall hazards greater than 10 feet.  The Alejandro Perez was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1485794, citation number 1, item number 1c and was affirmed as a final order on October 16, 2020, with respect to a workplace located at 3380 Noreen Drive, Columbus, OH 43221.
