29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is faing to ensure that the eyes of employees are protected from flying particles while working on a residential roof. This most recently occurred on or about October 03, 2019 on the jobsite located at 3130 104th Street, Omaha, Nebraska 68124.  Employees were observed using hand-held pneumatic nail guns to perform roofing operations on a residential home without any form of eye protection exposing them to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures. This was most recently documented at 3130 104th Street, Omaha, Nebraska 68107 on October 03, 2019 when employees were observed conducting roofing operations on a residential structure while working greater than 13 feet above the ground without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This event was most recently documented at the jobsite located at 3130 104th Street Omaha, Nebraska 68107 on October 03, 2019. Employees were using a twenty-four foot extension ladders to enter and exit the roof. This ladder did not have the side rails extended to at least 3 feet above the landing surface on the edge of the level of the roof. The ladder was extended approximately 2 feet above the eave. The ground to eave height of this house measured approximately 13 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer is failing to provide training to employees on the potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 3130 104th Street, Omaha, Nebraska 68124 where the employer has untrained employees re-roofing an existing residential home without the use of fall protection.
