The documentation did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test:  a.) Pennsylvania American Water, 279 Caa Lane, Clarion, PA - On or about October 4, 2019, and at times prior thereto: inspection, testing, and preventive maintenance records  for the System Control and Data Acquisition (SCADA) system had not been developed;     b.) Pennsylvania American Water, 279 Caa Lane, Clarion, PA - On or about October 4, 2019, and at times prior thereto: Records for daily visual checks of the chlorine system had not been developed; and    c.) Pennsylvania American Water, 279 Caa Lane, Clarion, PA - On or about October 4, 2019, and at times prior thereto: testing records for the chlorine sensor did not include the results of the inspection or test.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
