29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily:  a) On or about 10/09/2019 in the dry goods warehouse, employer did not ensure the clamp truck was inspected daily before being placed in service, exposing employees to struck by hazards due to malfunctioning of the clamp truck.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) On or about 10/12/2019 in the farmer's packing room, employee was working on the Dry Curd line and suffered a fingertip amputation when the door for the chute closed and it caught her finger between the door and the side of the chute. The chute did not have a guard preventing access to the pinch point.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1904.39(b)(2)(viii): Employer did not provide an accurate description of the work-related incident.   a) On or about 10/02/2019, employer did not accurately report a work-related fatality where an employee was fatally struck with a clamp truck. Employer initially reported the incident as a heart attack.        NO ABATEMENT CERTIFICATION REQUIRED
