This exposed employees to a fall hazard of approximately fifteen (15) feet on the West and South side of the structure. (b) On or about October 10, 2019, employees conducting roofing work on a flat roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately twelve (12) feet on the West, South, and East side of the structure.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) On or about October 10, 2019, the portable Werner extension ladder erected on the South side of the Structure, and being used for access to the porch roof, did not extend at least three (3) feet above the edge, and/or was not secured to the structure when it was not sufficiently extended. (b) On or about October 10, 2019, the South portable Werner extension ladder erected on the East side of the Structure, and being used for access to the roof, did not extend at least three (3) feet above the edge, and/or was not secured to the structure when it was not sufficiently extended. (c) On or about October 10, 2019, the North portable Werner extension ladder erected on the East side of the Structure, and being used for access to the roof, did not extend at least three (3) feet above the edge, and/or was not secured to the structure when it was not sufficiently extended.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired. (a) On or about October 10, 2019, the North portable Werner extension ladder erected on the East side of the Structure and being used for access to the roof, was not taken out of service, and/or tagged and marked do not use, when ladder was unserviceable and/or defective. This exposed employees to a fall hazard in-that the ladder had broken rungs.
