29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about July 28, 2023,  at the job site located at 7419 N Port Washington Rd., Glendale, WI 53209, employees installing gutters while on a roof with a 3:12 pitch were not protected from falling to the lower level.  This exposed employees to a fall hazard of approximately nine (9) feet.    Swift Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1638502, citation number 1, item number 3 and was affirmed as a final order on May 24, 2023, with respect to a workplace located at 1930 W Walnut Street, Milwaukee, WI 53205.  Swift Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1577606, citation number 1, item number 1 and was affirmed as a final order on August 4, 2022, with respect to a workplace located at 3360 S. Kinnickinnic Ave, Milwaukee, WI 53207.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) On or about July 28, 2023,  at the job site located at 7419 N Port Washington Rd., Glendale, WI 53209,  a portable ladder being used for access to the roof of a residential house did not extend at least 3 feet above the upper landing area.  This exposed employees to a fall hazard of approximately nine (9) feet.  Swift Roofing LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1)), which was contained in OSHA inspection number 1436925, citation number 1, item number 5 and was affirmed as a final order on April 2, 2020, with respect to a workplace located at 5410 W. Wells St., Milwaukee, WI 53214.
