1910.212(a)(1): Machine guarding was not provided to protect the operator and other employee(s) in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards.    (a) Mogul starch conditioning Machine and feed system: On or about October 11, 2019, the employer did not ensure barrier guarding, interlock and /or light curtains were properly implemented as part of the integrated safety system. This allowed the employee(s) to access the product tray conveyor in an energized state, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury.    (b) Mogul starch conditioning Machine and feed system: On or about October 11, 2019, the employer did not ensure barrier guarding, interlock and /or light curtains were properly implemented as part of the integrated safety system. This allowed the employee(s) to access the semi-finished product tumblers with conveyor belt in an energized state, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury.    (c) Mogul starch conditioning Machine and feed system: On or about October 11, 2019, the employer did not ensure barrier guarding, interlock and /or light curtains were properly implemented as part of the integrated safety system. This allowed the employee(s) to access the product bucket elevator in an energized state, exposing the employee(s) to caught in, caught between, ingoing nip point, crushing hazard and amputation injury.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:    (a) Mogul starch conditioning Machine and feed system: On or about October 11, 2019, the employer did not ensure barrier guarding enclosed the sprocket wheel and chain assembly that drives the product tray conveyor, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury.     (b) Mogul starch conditioning Machine and feed system: On or about October 11, 2019, the employer did not ensure barrier guarding was properly implemented This allowed the employee(s) to access  sprockets wheels that drive  the product bucket elevator in an energized state, exposing the employee(s) to caught in, caught between, ingoing nip point, crushing hazard and amputation injury.    (c) Mogul starch conditioning Machine and feed system: On or about November  14, 2019, the employer did not ensure barrier guarding enclosed the sprocket wheel and chain assembly. This allowed the employee(s) to reach into the sprocket and chain driving the tray stacker in an energized state. This exposed the employee(s) to caught in, caught between, ingoing nip point, crushing hazard and an amputation injury.    (d) Mogul starch conditioning Machine and feed system: On or about November  14, 2019, the employer did not ensure barrier guarding enclosed the sprocket wheel and chain assembly. This allowed the employee(s) to reach into the sprocket and chain driving the tray stack puller in an energized state. This exposed the employee(s) to caught in, caught between, ingoing nip point, crushing hazard and an amputation injury.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    (a)  On or about  October 11, 2019, employees working at, or around the product bagging machine were exposed to an unguarded 7/8" inch diameter shaft protruding approximately 4 inches past the bearing.  This exposed employees to a caught on and snagging hazard.
