29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about March 8, 2021, at the location of 3776 Kettle Court East, Delafield, WI, the employer did not ensure hazardous energy control procedures were developed, documented and utilized for equipment such as, but not limited to, the Viking Masek packaging machine. Employees, including maintenance personnel, perform servicing and maintenance work on equipment including, but not limited to, the Viking Masek packaging machine without the implementation of specific procedures, thereby exposing workers to amputation and laceration injuries. (b) On or about March 1, 2021, at the location of 3776 Kettle Court East, Delafield, WI, the employer did not ensure hazardous energy control procedures were developed, documented and utilized while employees were in the process of clearing jams and troubleshooting on the Viking Masek packaging machine, thereby exposing workers to amputation and laceration injuries.
