29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a) Pamarco Global Graphics/Roselle, NJ facility- Throughout the warehouse: Hyster forklift trucks with a non-factory-sling attached to the forks are used in the warehouse.  The employer did not obtain manufacturer's written approval before installing the sling for free rigging.    - Hyster SpaceSaver (Model #S50XL)    Condition occurred on or about 9/7/2019.

Caution shall be exercised when handling unstable load.    a) Pamarco Global Graphics., Roselle, NJ/ AST room- The employer allowed the employees to rig 750 lbs steel roll below-the-tine lift of a forklift truck. The unstable load resulted in the load slipping out of a sling and crushing an employee's foot.     Condition occurred on or about 9/7/2019.

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service.    a) Pamarco Global Graphics, Roselle NJ- The employer did not ensure slings were inspected for defects before use by a competent person.     Condition occurred on or about 9/7/2019 - 10/15/2019.
