29 CFR 1910.28(b)(2)(i): The employer must ensure each employee in a hoist area is protected from falling 4 feet (1.2 m) or more to a lower level by (a) a guardrail system; (b) a personal fall arrest system; (c) a travel restraint system:     On or about October 15, 2019 employees engaged in hoisting roll sections onto the mezzanine on machine B7 were not protected from the seven ft. fall hazard while standing in the hoisting area.

29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11):    On or about October 15, 2019 the guardrail system at the ends of the platform employees accessed to change product rolls was not equipped with a top rail 42 inches, plus or minus 3 inches, above the working surface, exposing employees to a fall hazard into running equipment below.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:     a) On or about October 15, 2019 the in-running nip point created by the moving conveyor belt and the roller was not guarded on machine 10 MOD 70, close to the plexy-glass into MOD 80, to prevent employee exposure to a nip point hazard.     b) On or about October 15, 2109 the in-running nip point created by the moving conveyor belt and the roller was not guarded on machine 10 MOD 70, close the adjustment bars in the folding operation, to prevent employee exposure to a nip point hazard.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself:  a) On or about October 15, 2019 the two expanded metal fixed guards installed under the conveyor belts in MOD 70 on line 10 created a shear hazard with jagged and/or sharp edges.   b) On or about October 15, 2019 the expanded metal fixed guard installed above the splicer in MOD 70 on line 11 created a shear hazard with jagged and/or sharp edges.
