Employees were exposed to crushing and amputation injuries while working within two to three feet of the Weiler-05's beef auger where the auger's mixer tank not completely covered.  b. Employees were exposed to crushing and amputation injuries while working within two to three feet of the Weiler poultry auger blades where the auger's mixer tank did not have a cover.  c. Employees were exposed to pinch hazards while directly loading blocks of frozen beef onto the vertical conveyor at Grinder-05 with a broken weight plate at the loading chute.
