(a) On and before October 17, 2019, on the Schilling fk6 Panel Saw in the CNC production area located at 809 Philip Dr., Waukesha, WI 53186, where employees use the saw to cut a variety of materials, interior surfaces were covered with an adequate layer of resin and wood dust to facilitate ignition by the heated blade motor. The dust accumulations resulted in smoldering combustion events, and exposed employees to fire hazards and inhalation of toxic chemical hazards.
