29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition. (a) On and before October 17, 2019, on the Schilling fk6 Panel Saw in the CNC production area located at 809 Philip Dr., Waukesha, WI 53186, where employees use the saw to cut a variety of materials, interior surfaces were covered with an adequate layer of resin and wood dust to facilitate ignition by the heated blade motor. The dust accumulations resulted in smoldering combustion events, and exposed employees to fire hazards and inhalation of toxic chemical hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) On or about October 29, 2019, at the workplace at 809 Philip Dr., Waukesha, WI, 53186, at the I- beam support between the storage racks and the insulation lamination area, a flexible cord was used as a substitute for fixed wiring to supply power to a scissors lift used by maintenance personnel, thereby exposing employees to electrical shock and fire hazards.
