29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about July 29, 2020, employees starting up the Sheet Pultrusion Line were exposed to caught-in and crushing hazards created by the automatic pullers. The employer did not ensure the lines' two puller point of operation were properly guarded. The Gund Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1240110, citation number 1, item number 1, and was affirmed as a final order on November 30, 2017, with respect to a workplace located at 809 Philip Drive, Waukesha, WI 53186.
