29 CFR 1910.146(d)(8): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not designate the persons who were to have active roles in entry operations and did not identify the duties of each such employee in those roles. On or about October 29, 2019 at the Leggett & Platte Inc. Facility- Under the permit-required confined space program it was discovered that the program was deficient in designating the persons who were to have active roles in entry operations and identifying the duties of each employee in those operations.
