29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a) On or about 11/06/19 in Compressor Room #3: employees who inspect and service equipment on the mezzanine were exposed to a fall hazard of approximately 12 feet when a majority of the area on the SW side of the receiver was protected with diagonal structure supports with opening as wide as 32 inches at a height of 32 inches.          b) On or about 11/06/19 in the Compressor room #3: employees who inspect and service equipment on the mezzanine were exposed to a fall hazard when the area on the SW side, at the end of the receiver on the smaller work platform, was open ( 30 inches wide ) and without fall protection.   c) On or about 11/06/19 in Compressor Room #3: employees who inspect and service equipment on the mezzanine were exposed to a fall hazard of approximately 12 feet when a majority of the area on the NW side of the receiver was protected with diagonal structure supports and the remaining area was protected only by a top rail at a height of 42 inches nominally.      d) On or about 11/06/19 in Compressor Room #4B: employees who inspect and service equipment on the work platform adjacent to the VS403 transfer vessel, were exposed to a fall hazard of approximately 14 feet when the platform lacked a mid rail on the guard rail system.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.  a) Compressor Room #3 - On or about 11/06/19, the employer did not document the two pipes used for water lines ( located on the NW wall near the ceiling) penetrating the interior wall of the machinery room, complies with the employer's chosen RAGAGEP such as, but not limited to, Sections 8.12(f) of ASHRAE 15-2010 and ASHRAE 15-2013, when the wall penetrations were not tightly sealed exposing employees to the hazards of ammonia in the event of a release in this area.    b) Compressor Room #3 - On or about 11/06/19, the employer did not document that the steel braided hose hanging on the east wall complied with the employer's chosen RAGAGEP such as but not limited to, Sections 9.1.1 of ASHRAE 15- 2010, and the employer's Ammonia Hose Safety Policy, when there were no obvious markings or tags on the hose to indicate suitability for ammonia service and no documentation to indicate whether the hose was compatible for ammonia service, exposing employees to the hazards of being sprayed with ammonia.       ABATEMENT CERTIFICATION REQUIRED
