29 CFR 1910.30(a)(1): Before any employee was exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart: On or about October 31, 2019, the employer did not establish a training program to address fall hazards as required in particular for the worker(s) in the cobalt II building who were accessing the top of a T903 holding tank with the use of a portable ladder.
