29 CFR 1910.102(a): In-plant transfer, handling, storage, and/or use of acetylene in cylinders did not comply with the provisions of CGA Pamphlet G-1-2009 ("acetylene"):  (a)  On or about November 13, 2019 and at times prior to that date, for the employee exposed to potential fire and explosion hazards where the employer did not ensure an acetylene cylinder, not in use, was properly secured in a location which would prevent accidental contact or damage at or near 50 Bastian Road, Battle Mountain, Nevada.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about November 13, 2019 and at times prior to that date, for the employee exposed to amputation and/or caught-in hazards from rotating machine parts while operating the Delta Drill Presses located in the maintenance shop and woodworking shop located at 50 Bastian Road, Battle Mountain, Nevada.
