29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Work Shop: The left side adjustment between the tongue and the abrasive wheel of a DeWalt DW758 8" bench grinder exceeded one fourth inch. There was a measured gap of 1.125 inches. This condition potentially exposed employees to caught-in hazards.

29 CFR 1910.243(c)(3): Abrasive wheels used on right angle head grinders were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: Tool Shop: Two DeWalt angle grinders were not equipped with safety guards having a maximum exposure angle of 180 degrees. Employees were potentially exposed to flying objects which can cause struck-by injuries.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. On or about March 12, 2025 and at times prior thereto, two relocatable power taps were being used in a manner inconsistent with the listing and labeling for such items. 1. Records / Mail Room: An orange extension cord was plugged into a relocatable power tap near a desk. 2. Warehouse / Fire / Fire cache: A relocatable power tap was affixed to the wall using a metal bracket and screws. These conditions can cause overloading of circuits or fires. Employees were potentially exposed to electric shock hazards.
