29 CFR 1926.251(a)(2)(i): The employer did not ensure that rigging equipment had permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load: a) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306 A subcontractor's employees were exposed to struck by hazards as they were working on the 70th Floor with loads rigged by wire ropes that did not have the appropriate load capacity tags on them. Violation was observed on November 20, 2019.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306 Employees were exposed to an approximately ten foot fall hazard as they were working in the area of 70th Floor Staircase A with open floor holes without effective floor hole covers in place to prevent them from falling to the concrete floor below. Violation was observed on November 20, 2019.
