29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a. On or about November 25, 2019, at the above addressed jobsite, an employee was carrying material up an extension ladder, thereby exposing the employee to hazards associated with falls.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  a.  On or about November 25, 2019, at the above addressed jobsite, employees performing commercial roofing operations were not protected from falls of three (3) stories, thereby exposing employees to fall hazards.  R & R Construction Services, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1398180, citation number 1, item number 1 and was affirmed as a final order on July 15, 2019, with respect to a workplace located at 1971 Berkshire Pl, Wheaton, IL 60189.
