The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 9'3'' above a lower level was protected from falling by one or more of the following: guardrail system, safety net system, personal fall protection system    a)  Reddy Ice Corporation, 9670 E. 40th Ave., Denver, CO  80238:  On and before November 25, 2019 the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 9'3'' above a lower level was protected from falling by one or more of the following: guardrail system, safety net system, or personal fall protection system.  An employee was changing out a suction head on a robotic bag palletizer known as "Betty" without the use of fall protection.  This condition exposed employees to fall hazards.

Employees work on robotic bag palletizers during maintenance activities while energized.  This condition exposes employees to struck-by hazards.

The employer did not designate and train employees to assist in a safe and orderly evacuation of other employees:    a)  Reddy Ice Corporation, 9670 E. 40th Ave., Denver, CO  80238:  On and before November 25, 2019, the employer did not designate and train employees to assist in a safe and orderly evacuation of other employees. Ammonia alarms went off in early December for calibration and testing.  Employees didn't know what was going on and continued to work.  This condition exposed employees to fire hazards.
