29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about November 15, 2019, at the jobsite located at 850 West Park Road, Union, Missouri, the employer did not ensure procedures for the control of hazardous energy sources were developed, documented and used where equipment operators were required, to lockout equipment in the file imprint system. Employees were exposed to amputation hazards where they were instructed to use the stop safe button prior to reaching into the machine/point of operation.
