29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   At the roof of a residential community clubhouse in construction, located in Lot 1 of Jensen Beach Village, Jensen Beach, Florida, 34958: On or about November 29, 2019 and at times prior, the employer failed to implement frequent and regular inspections of the job site, thereby exposing employees to fall hazards from a roof and from a ladder.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the side of a roof of a residential community clubhouse in construction, located in Lot 1 of Jensen Beach Village, Jensen Beach, Florida, 34958: On or about November 29, 2019 and at times prior, employees were exposed to a fall hazard of 9-feet, 4 inches to ground level, in that; the side rails of a ladder did not extend at least 3-feet above the landing surface of a roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    At the roof of a residential community clubhouse in construction, located in Lot 1 of Jensen Beach Village, Jensen Beach, Florida, 34958: On or about November 29, 2019 and at times prior, employees were exposed to a fall hazard of 9-feet, 4-inches to ground level, in that; no fall protection was being used when employees were performing work on a roof with a 5/12 pitch.  CJM Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1235090, citation number 1, item number 1 and was affirmed as a final order on 01/11/2018 with respect to a workplace located at 11607 SW Rowena Street in Port Saint Lucie, Florida.
